Our dedicated, experienced and qualified team provides daily support, therapeutic care to provide the highest level of care possible to our young people. We work together with a team of therapists and councillors to enable us to form an understanding of the internal world of our young people using state of mind assessments

Our therapists and councillors provide a range of services including, assessments, psychotherapy, observational studies, play, music and drama therapy, psychodynamic counselling, meditation, humanistic counselling and liaison with CAMHS and other agencies. They are highly experienced, qualified and support our team by providing their expertise and knowledge enabling reflective work to be conducted, ensuring consistency, offering guidance and making certain that individual plans and goals are achieved. Other specialist support services are implemented based on individual care needs

Referral

To make a referral please contact us on info@spiresquared.com or call 0333 772 9213. Our team have full knowledge about our service and the potential challenges faced by the young person and the support required for an increasingly diverse community. Once we receive the referral we will complete a pre-admission person centered support planning to review the young person’s needs, abilities and risks. This will determine the best possible outcome ensuring a seamless and successful referral, assessment and transition towards living independently.

Assessment

We create a safe space for young people to release, to express and to connect. Our teams work towards identifying their needs, aspirations and their challenges to empower them to create a positive impact within society. Assessments underpin our care pathway, when we receive a referral, the young person’s care needs are reviewed. If we believe that we are able to provide the right support and treatment, we will be in-touch to complete our person centred support plan which is designed for individuals to achieve the most positive outcomes. The assessment process is in-depth to ensure that all involved in the individual’s care will have confidence that the placement will meet their individual needs and be a success. Transition

Once the assessment and initial visit is complete and it’s agreed that the placement will meet the individual’s care needs, we will provide a costing to the commissioning authority to be reviewed by the funding panel. If agreed, the placement is confirmed.  Each Local authority has their own eligibility criteria, based on the national criteria outlined in the Care Act. They Complete a Care Needs Assessment (CNA) to discuss your care and support needs and help you maintain your well-being and independence. The decision is based on the results of the assessment. The local authority confirms how it will meet your  needs in a Care and Support Plan, which sets out what services will be provided and when they will be provided.

Funding

Once the placement and funding is approved we begin the transition process which will be as seamless as possible and individuals understand the continuum of their care. Firstly we will arrange a discharge planning meeting and a transition plan which will include the service manager, family and anyone else that is involved in the young person’s care. We plan the transition together so that our young person is as comfortable as possible.
